Is it possible to avoid extractions? (Class 2)

Hello!

My orthodontist recommended surgery to include my case so I had an appointment with two different surgeons.

The first 1 didn't mention extractions at all. He talked about a counterclockwise wise rotation with biomaterial grafting and a genioplasty.

The 2nd surgeon talked about a BSSO with genioplasty but only after the orthodontist has decompensated both jaws. He said extractions were mandatory because my lower incisors are tipped forward and could even fall.

Currently my teeth are aligned due to camouflage orthodontics when I was 12. But it's true that my lower incisors are tipped forward and my bite has slowly been opening again through the years (I'm 32). It's very mild, maybe 1-2mm.

What do you think?
Why are both surgeons suggesting something completely different? Does counterclockwise rotation allow the preservation of the premolars?
