Insurance denial

Blue Cross denied coverage of my surgery because I am older than 19, and they say the policy only goes up to 19. Is it worth appealing this? Does anyone have tips on how to file a good appeal?

In my experience, the surgeon sent a letter directly to Blue Cross. He said the best way to avoid a denial is to explain the purpose of the surgery and make it clear it's only for medical conditions associated with the open bite. I had no issues, they paid on time, and actually they send a confirmation letter a year before the surgery to my surgeon. I got a copy of the letter from them as well. My surgery was 2 years ago and I was 43 years old. Perhaps check with your surgeon and ask why they deny you based on age. It seems to me like a crazy reason for the denial. Good luck.
