Need help with braces

Hi all! I've been wearing the metal braces for about 1.5months so far. I've gotten it due to multiple gaps between my teeth, which are deemed small by almost every dentist I've visited. There have been a few problems so far and I've been hesitant to ask my dentist directly as I don't wish to seem to be rude.
1) Do all cases regarding braces need to be reviewed by an orthodontist? I recently found out about the differences between an orthodontist and a dentist. Turned out that they used to have both, but the orthodontist has since left the Clinic. Will that affect my case a lot?
2) Today, another bracket fell out again, and this has happened two times so far. I used to be a teeth grinder but it had stopped when I put on my braces. Thus I was shocked when it fell out when I woke up. The first one fell out when I was eating soft chicken and rice. I don't mind the bite blocks but I realised that they get worn down very quickly. Should I ask the dentist to put on a larger amount of clay to prevent another bracket from falling out? What are the other things I can do?
