I've had the herbst appliance. Its not too bad at all. I needed to have it because my jaw was so far back and they needed to bring it forward alot, or else i would have had to have surgery. I had it for the first year or so of my treatment ( i just got my braces taken off on thursday! =D) but anyways. the only thing that i didnt like about them was that whenever i ate something sugary, or salty sometimes it would like get stuck in the appliance and it was weird. I also played with them a lot taking them apart all the time.

I have a removable herbst appliance. I use it for sleep apena and have done real well with. I have had it for a year now. I did have a sleep study follow up and it does work real well. I was able to go off the CPAP mchine and just use the retainer. If you have any questions just ask. I did start out slowly with adjusting it every few days. I would turn it a half turn each time. Took about two-three weeks to move the lower unit out.
