I think this can be fixed by the orhodontist.... or maybe he's considering the space between my teeth in the equation and has already put that in his equation... I dont see how he could've missed this.

once the molds are done they have to do the surgery soon after, they can't make ortho adjustments and if the teeth move it botches the molds and they'd have to take new molds and face bow and model surgery and all that bs.

I'd go with the out of network surgeon and the in network hospital, I would not go with a guy with 4 yrs experience--but it would also depend on who he learned from, you should ask that. 101 is a high fever... My surgeon said fever is no good. call your doctor and ask for a keflex script and clorhexidine 0.12% rinse. I personally feel you're suffering when you don't have to and your surgeon doesnt give a crap.
