I haven't had a general anaesthetic but I have had a couple of day procedures with sedation whilst I had lower braces with lower FRLA and an upper expander. The expander was fairly heavy duty, all over the roof of my mouth - TPA banded to my first molars, joined to a Nance banded to my first premola...

I've had both - I had metal upper & lower the first time (as an adult), & have ceramic upper & metal lower this time. I definitely felt like all metal was in the minority on AW the first time. When I had traditional metal brackets (non-SL) my powerchains were always applied in front of the wire. I had them for most of my 18mth treatment, I never had one break, nor did anyone ever mention one might do so.
