My insurance has told me that surgery for my overbite is not covered ("excluded") based SOLELY on the fact that it's "developmental" and not "congenital" (as in, not present at birth). I would file again and make sure it goes to the medical approval department. This might just one of the typical hurdles insurance sets up, hoping most applicants will give up.
