There is an active thread on Dr. Bloomquist, a few lines below your subject. I believe he is one of the affordable, out-of-town doctors if you are paying privately. If I'd had to pay out-of-pocket, I would have tried him first.

luke: are you able to drink from a paper cup yet? I have found those the easiest because they can be bent a little.
