What teeth aren't showing when your smile? If your uppers are reclining then he's unlikely to recline them further.
Discuss with your orthodontist about ways to bring the lower jaw forward.

Stay away from the lookism type sites, some pseudo/bro science with very little basis of long term impact.

A sliding genioplasty won't impact your teeth/bite, so it's definitely possible to have it with braces on.

It will impact the soft tissue position so perhaps could change your orthodontists approach to smile aesthetics? i.e. ask your orthos opinion.

Sounds a bit like a cross bite. Get a consultation from an orthodontist, you can often get free consultations.

Do you favour one side when chewing? That may explain the perceived muscle imbalance; of course you may be favoring that side due to your bite.

What are you eating when they fall out? :D

Sometimes your teeth will be too tight for the spacers to function properly, if the spacers don't work they can always use buccal tubes and then switch to bands (if needed) later in the treatment

What will happen with your postop orthodontic care? Will the NHS still do it? It's not rare to have elective procedures postponed on the NHS if there's a emergency case or even due to a lack of available beds, I'd imagine private operations get postponed far less frequently. I'd be a little concerned over an apparent lack of focus on your lower jaw. There are appliances around such as the Herbst/Forsus that will help bring your developing lower jaw forward.
Try and get some more consultations.

I was just going to have braces treatment going down the private route, costs was around £5k. £7k does seem rather cheap for surgery, was that one for one jaw?

I consulted with a few private orthos and they glossed over "to get a perfect result" you'd need surgery, but I had no inclination that it might be available on the NHS. Obvious advantage is lower treatment time i.e. you miss the prep phase, and the teeth tend to move quicker after jaw surgery.

Certainly not common but it's sometimes indicated

Buccal tubes are fairly common. I've got molar tubes on top and buccal tubes on the bottom, mainly because they couldn't create enough space on the bottoms to bands in.

The only issue is that the wire occasionally pops out of the buccal tubes which doesn't happen with the molar bands.

Your results look like a significant improvement. However you do appear to have a forward head posture, if you don't get that sort then you're more likely to relapse.

Early in treatment it's unlikely to have much impact. Push on the bend with your thumb and it will probably straighten.
